  • Salary: £30,000 a year.
  • Job type: Full-time, permanent.
  • Shift and schedule: Monday to Friday with weekend availability.

Job description

As a care manager, you will be responsible for:

  • Receiving referral information and liaising with referring individuals in relation to provision of live-in care for clients.
  • Reviewing complex and non-complex care plans and risk assessments for new and existing clients respectively.
  • Gathering relevant information and write Care Plans for new clients.
  • Conducting quality assurance checks with clients and/or their advocates to ensure appropriate care is being provided and identify any changes in care needs.
  • Participating in the on-call rota providing cover for urgent enquiries during evenings and weekends (remunerated).
  • Recording client information on the electronic management system, New Care and PASS (EveryLIFE).
  • Managing the on-going involvement with your clients’ care needs & liaising with all professionals.
  • Building strong relationships with your carers.


  • Car driver with clear licence and own car, willing to drive to clients’ homes.
  • Prior experience in an office-based environment, within the care sector.
  • Must be a team player.


  • Organised, methodical, and with a good eye for detail.
  • Proficient with computer systems.
  • Excellent telephone manners.
  • Confident & presentable with face to face interaction.
  • Ability to work under pressure.
